Billing Information

EMG and Rehabilitation physicians accepts most insurances, but YOU the patient need to call your insurance company to make certain of that. If for some reason you are unable to perform that duty please contact our billing office at 937-438-9500 and they can assist you.

Billing for Dr. Petty, Dr. Braunlin, Dr. Kronberg, and Dr. Hinman is handled by Maxx Medical, Inc.  Maxx Medical Inc. will file all the insurance claims for you, primary, secondary and tertiary. All we require is an updated insurance card so that they can file appropriately. Maxx Medical Inc. accepts cash, checks, Mastercard and Visa.

Billing for Dr. Richard Beers is handled by Verne Glaser & Associates, Inc. 937-298-5536.  Verne Glaser & Associates, Inc. accepts cash and checks only.

Co-pays and deductibles are due at the time of service. If you do not have insurance we are willing to set up a payment plan that is feasible for you, just contact us.
