Information for Referring Physicians

Thank you for exploring our website. Whether this is our first introduction or we have been working together throughout the last 25 years of our practice life, we appreciate the opportunity to share in the care of your patients.

EMG & Rehabilitation Physicians is a group of specialists in Electrodiagnostic Medicine and Physical Medicine and Rehabilitation. We are dedicated to providing comprehensive, compassionate, and professional care to our patients. All physicians in our group are board certified in the specialties of Electrodiagnostic Medicine and Physical Medicine and Rehabilitation. We have practice locations at Kettering Medical Center, Sycamore Hospital, and Clinton Memorial Hospital in Wilmington.

To simplify the process of referring a patient, please download and use our Patient Referral Form. Please include Patient’s current and past medical history, recent progress notes, list of current medications and the most recent MRI/CT Scan/X-Ray reports.
