To prepare you for your upcoming visit, please be aware of the following:

Please pre-register for your appointment by calling 937-395-8880
On the day of your EMG appointment, please do not use any skin lotions or oils
Please bring your completed questionnaire to your appointment
Click here to download any required forms (no questionnaire is necessary if you are being seen for EMG only).
Please bring your current insurance card and drivers license to your appointment
If your appointment is related to an auto accident or Workers Compensation claim, we must know in advance.
